Trainee Forms

Use the forms listed below in the order presented on the right to make enquires as an Employer or Trainee.  By following the process from top to bottom you will be able to progress from enquiry, to enrolment and finish with your qualification.

• The Request Information Form provides an initial enquiry form.
• The Employer Information Request Form allows you to supply Employer Information to DTW Designs to assist in the management of client training.
• The Student Enrolment Form allows you to supply DTW Designs with your personal details for enrolment in the 30498QLD Course in Operating Pressure Equipment and/or the Steam Power & Co-Generation Course.
• The RPL Application Form allows you to apply for RPL in the 30498QLD Course in Operating Pressure Equipment with DTW Designs as the Registered Training Organisation.
• The Workplace Assessment Request Form allows you to request Assessment for WHS Certification on completion of your Formal and Informal Training.
• The Workplace Assessment Feedback Form allows you to comment on the assessment and results provided by the Assessor on the WHS Assessment Summary Sheet.
